Fight the Flu with UPEA – 2021 Flu Shot Clinic Schedule

The Utah Public Employees’ Association will once again host flu shot clinics in partnership with Community Nursing Services (CNS). These clinics are open to all state employees and their families. If you are insured by PEHP, there will be no cost or co-pay required. If you have a different provider, please bring your health insurance card and photo identification. CNS will bill any of its accepted insurance providers. The health and safety of employees and their families is our priority; therefore, certain precautions must be taken for you to attend:

  • We ask that Everyone in attendance, even those who are fully vaccinated, wear a mask and make an effort to maintain social distance.
  • Employees are not required to reserve a time slot for any of the UPEA clinics this year – However, if the room becomes too crowded, you may be asked to wait outside or complete your paperwork elsewhere before returning to get the vaccine.
  • For the health and safety of others, please follow current guidelines regarding COVID-19 quarantine and isolation. If you have tested positive for COVID-19 in the past 10 days, or you’re experiencing flu-like symptoms such as fever, cough, sore throat, etc., please stay home.

The following schedule is subject to change. If you have any questions, please contact Kendle Zdunich at
