If any of your contact information has changed since you joined UPEA, you may no longer be receiving communications specifically curated to your district, agency, or region.
Please complete the following form to verify that UPEA has your current contact information 

Utah Dept. of Workforce Services, SLCo Public Works, Utah Department of Corrections, etc.
Building name (if applicable), address, city, zip
Any other information you'd like UPEA to update in your membership file.
Old phone numbers, emails, etc. - These will be removed from your membership file.
